If you have problems with your computer when 'Submitting' the form to us, simply complete the form as normal by filling in all of the boxes, then print it out and post it to us with your deposit.

Use the 'RESET' button to clear the form.

Please enter into the box below the Course Title/s and date/s that you wish to attend, clearly state if you will be attending the whole course, or indicate which days you will be attending.

If this is your first course/s with us at Ershamstar, let us know a bit about yourself, such as your level of ability, your psychic or mediumistic experiences, do you sit in a Circle, etc...... etc ...... We give this information to the course tutor so that they can arrange groups that match each other as best as possible.

If you wish to provide any further information or you wish to ask any questions that may require a reply, please enter it into the box below. You may use this box to write anything concerning your booking with us.

After you have sent this form to us by clicking on the SUBMIT button, you will need to post to us a cheque made payable to "Mrs S French" , NOT Ershamstar.

You are required to send a deposit of £50.00 for each residential course place that you wish to reserve and £30.00 for a day student place. The balance must be paid at least 6 weeks prior to the commencement of the course, you will be reminded either by email or by post that the balance is due, please pay when requested, or you may lose your place.
